Bi-State

Bi-State Primary Care in Collaboration with all Participating Providers and Affiliates of Community Health Accountable Care:

This project’s goal is to increase provider collaboration across the continuum of care in local communities.  The objective is to grow and strengthen Community Health Accountable Care (CHAC), a Shared Savings Accountable Organization involving around 1,600 providers and impacting 35,300 attributed lives.  Under the sub-grant, CHAC has been cooperating with other ACOs in the region, taking part in the Unified Community Collaboratives associated with the Blueprint for Health, and participating in the VHCIP Learning Collaborative.  They have established an eight-member Consumer Advisory Panel, and they continually track health information for patients, including those who opt out of sharing data with the ACO.  They have designed a Quality Improvement Dashboard for the ACO, making it easier for providers to track patient care and for the organization to pull relevant data.  In 2014 CHAC generated savings under the Vermont Medicaid Shared Savings Program (VMSSP) and the Medicaid Shared Savings Program (MSSP).
