The project aims to intervene in tobacco, alcohol, and drug misuse by establishing Screening, Brief Intervention, and Referral to Treatment (SBIRT) in the patient-centered medical homes at Central Vermont Medical Center (CVMC).  SBIRT is a comprehensive, integrated public health approach to the delivery of early intervention and treatment services.  Screening in the Medical Homes (SiMH) aims to prevent and reduce substance misuse, reduce healthcare costs, increase care coordination, and enhance patient participation in recovery from substance misuse.  Once patients have been identified, they receive appropriate services and continuity of care throughout their change journey.  By the end of 2015, the project has screened 2,646 patients, providers have done brief interventions with 145 of them, and 86 patients have been referred to brief treatment.  All together, 66 patients have been engaged in SBIRT services, whether brief treatment or follow up care.
