Developmental Disabilities Council with Green Mountain Self Advocates:

The Institute for Health Policy and Practice is working to identify and recommend best practices in the delivery of health services to adults with intellectual and developmental disabilities (I/DD) in Vermont.  The project has engaged individuals with I/DD and their families in an inclusive planning process that bridges gaps in understanding between traditional medical services and those who either provide or receive Disability Long Term Services and Supports (DLTSS).  The project team has already collected and analyzed data describing the health status and care experience of Vermont adults with I/DD.  They are in the process of identifying and recommending practices that will improve those health care experiences and reduce the disproportionate burden of illness experienced by this population.  By sharing information and soliciting input, the project team builds relationships with other collaborative health care groups, including the Blueprint for Health and the VHCIP Regional Learning Collaboratives that are working toward more effectively integrating traditional medical care and DLTSS.
