Northeastern Vermont Regional Hospital

Northeastern Vermont Regional Hospital in Collaboration with Northern Counties Health Care, Rural Edge Affordable Housing, the Support and Services at Home (SASH) Program, the Northeastern Vermont Area Agency on Aging and Northeast Kingdom Community Services:

The Caledonia and Essex Dual Eligibles Project aims to reduce overall health care costs, make more efficient use of Medicaid special services, and improve the well being of clients in their region who are eligible for both Medicare and Medicaid. The project includes a Health Coaching service with an active caseload of 20 clients, which has reached a total of 65 Dual Eligibles.  The Health Coach has had two clients quit smoking.  The Health Coach has also helped a client start a Chronic Pain Support Group.  The project is connecting food-insecure clients to Meals on Wheels.  There is a partnership with the Lyndon State College exercise science program, offering free personal training for Dual Eligibles. For the long term, the project is working with the Vermont Food Bank to set up a mobile food pantry at the hospital.  The project also provides funds for goods and services not covered by health insurance, when the comparatively low cost of those items would make a significant difference in a person’s health.
