This project supports the Blueprint for Health goal of improving care for patients with chronic illnesses by integrating supportive care and end-of-life decision-making earlier in the care process. The project team aims to facilitate patient care decisions based upon patients’ own values. By avoiding unnecessary hospitalization for patients with complex conditions and needs, improving symptom management and quality of life for the patient and caregivers, and promoting earlier referrals to hospice, the project promotes a patient-centered care model for shared decision making.
