The Learning Dissemination Plan will translate findings from the Vermont Health Care Innovation Project State-led evaluation into real world language, visuals and tools that will impact the practice and perception of health care. This plan will identify ways to deliver evaluation findings to target audiences across Vermont that encourage them to incorporate evaluation findings into their work, language and, communication with other stakeholders toward sustainability of successful reform efforts.
