Sub-Grant Program

Using funds from the federal State Innovation Model (SIM) grant to Vermont, VHCIP has supported collaborative health care initiatives through a Sub-grant Program.  The SIM grant, which came from the federal Centers for Medicare and Medicaid Innovation (CMMI), provides money and other resources to support innovations in health care payment and delivery systems throughout the state.  The sub-grants have enabled interdisciplinary and cross-organizational groups of providers throughout the state to move toward our goal: improve care, improve the health of the population, and reduce per-capita health care costs by 2017.

One purpose of the Sub-grant Program is to involve as many entities outside of state government as possible in the effort toward achieving the Triple Aim.  The grants have targeted projects for transforming payment and delivery systems.  Projects from around the state have already made important progress.
