How can we better support Vermonters with complex health care needs? Providing coordinated, efficient, person-centered care for the most complex and costly cases is a critical step toward achieving the Triple Aim of improved population health, better patient experience, and reduced health care costs. Great progress has been made in this area, but there is still work to be done. During this 1-hour webinar, based on a recent report of the same name published by the national Center for Health Care Strategies (CHCS), participants will discuss opportunities in six domains: care model enhancements, financing and accountability, data and analytics, workforce development, governance and operations, and policy and advocacy.
