This group, formed in October 2015, built on the work and membership of the former Care Models and Care Management Work Group, as well as integrating members of the Population Health and DLTSS Work Groups. In Performance Period 3, the group was charged with:
- Monitoring implementation of Learning Collaborative, Sub-Grant program, Regional Collaborations, and Workforce Supply and Demand Data Collection and Analysis, and ensuring these activities are included in Vermont’s SIM Sustainability Plan as appropriate.
The group also recommended mechanisms for assuring greater consistency and/or coordination across these programs and models in terms of service delivery or other key model or program components. The goal was to maximize effectiveness of the programs and models in improving Vermonters’ experience of care, reducing unnecessary costs and improving health, and minimizing duplication of effort or inconsistencies between the models.
