These organizations received funding and other support from the federal SIM grant through VHCIP. Two years into the grant period, the projects have collectively had an impact on hundreds of providers and hundreds of thousands of Vermonters. Each project is focused on transforming payment systems, improving delivery systems, or both. Together, these public and private organizations are bringing the state closer to achieving the Triple Aim. (Click on the grantee for each project for a more detailed description.)
