RISE Coalition: Northwestern Medical Center in collaboration with all of Franklin County:
The goal of RiseVT is to increase the overall health of the population by decreasing the percentage of overweight and obese individuals. Toward that end, the project seeks to increase the number of employers offering a wellness program in which more than 50% of their employees participate. And finally, the project aims to expand resources for biking and walking in the region. The larger goal is to reduce the prevalence of such chronic diseases as cardiovascular disease, cancer, chronic obstructive pulmonary disease, diabetes, and asthma. To reduce the incidence of these diseases in the population, it is necessary to address risk factors like obesity that are modifiable. The project has been active on the level of individual clients, schools and businesses, municipalities, and social media to raise awareness of the RiseVT program. Through collaborations with the local mental health agency, Vermont Adult Learning, Franklin County Home Health Agency, the Agency of Human Services and more, RiseVT is reaching out to the region’s most vulnerable populations.
