Many population health improvement efforts are already underway in Vermont, administered through the Department of Health, the Blueprint for Health program, employers, hospitals, Accountable Care Organizations, Federally Qualified Health Centers (FQHC’s), local governments, payers, and providers. The Population Health Work Group examined these initiatives as well as VHCIP innovations, assessing their potential for improving the health of Vermonters. They also recommended ways in which VHCIP could better coordinate health improvement activities in order to have a more direct impact on population health.
The Population Health Work Group was engaged in:
- Enhancement of State initiatives administered through the Department of Health
- Support for local and regional initiatives led by governmental or non-governmental organizations, including employer-based efforts
- Expansion of the scope of delivery models to include population health, within both pre-existing state initiatives and innovations recommended by VHCIP
